CONTINUUM OF CARE

The “Discovery” in Discovery House is about the programs, services, and innovative therapies that help women and children heal and rebuild their lives.

Each individual client experiences a continuum of care to meet their needs. Every family is assigned an adult intensive case manager, children & youth case manager, mental health clinician, and housing liaison. Women and their children have barrier-free access to our evidence-based and trauma-informed programs and services. All Discovery House staff are Brain Story Certified through the Palix Foundation and all staff receive regular and extensive training on trauma-informed practice.

ADULT CASE MANAGER

Adult case managers work with women to ensure their basic needs are met. They also provide danger assessments and safety planning, advocacy, legal support, referrals, and life skills coaching.

MENTAL HEALTH CLINICIAN

Mental health clinicians (RSW, MSW, and psychologists) provide families counselling, as well as resources to recover from physical, emotional, and psychological issues.

CHILD & YOUTH CASE MANAGER

Child trauma specialists work with children to address exposure to abuse and poverty, coping skills, emotional regulation confidence, and healthy relationships.

HOUSING LIAISON

The housing liaison works with the families to ensure they have appropriate housing and resources to meet their needs.

CLIENT IMPACT

Discovery House’s continuum of care helps women and children heal from their trauma and achieve self-sufficiency. Our work helps children achieve developmental milestones, improve mother-child attachment, and increase resiliency. Families are equipped to transition out of the program into independent housing, maintaining stability and living free from domestic violence.

 

COMMUNITY IMPACT

Our evidence-based programs help break the cycle of intergenerational violence for hundreds of children and youth each year.
